Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
An important note about DOT SAP services
SAP services are performed under federal regulation. Part 40 requires the SAP to provide specific reports to your employer's designated employer representative, and, for FMCSA-regulated drivers, to record certain information in the Drug & Alcohol Clearinghouse. These disclosures are required and are not optional.
Uses and disclosures
We use and disclose health information to provide services to you, to obtain payment, for our own operations, and where law requires. Disclosures required by 49 CFR Part 40 are described above.
Disclosures requiring your authorization
Other disclosures are made only with your written authorization, which you may revoke in writing at any time, except to the extent we have already acted on it.
Your rights
You may request access to your record, request an amendment, request an accounting of certain disclosures, request restrictions on certain uses and disclosures, request confidential communications, and receive a paper copy of this notice.
Substance use disorder records
Where records of substance use disorder treatment are subject to 42 CFR Part 2, those additional protections apply and generally require your written consent for disclosure.
Complaints
You may complain to us at info@nsurogroup.com, or to the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
Our duties
We are required by law to maintain the privacy of protected health information, to give you this notice, and to follow the terms of the notice currently in effect.
